I would have it notarized because in the event of an emergency, the hospital might need something that has a notarized signature.

I don't think you need to get it notarized. When my neice was little, she lived with my mom for 6 months and my mom would take her to the doctor and once to the ER with just a note from the parents giving her permission for medical care. They never gave her a problem, they would just photo copy the note and put it on file.
ETA: that was 4 years ago, so i am not sure if anything changed.

I had to have my dad take Aly to the dr once and I just sent a letter signed by me...I dont think you need anything notarized at all.... have a great trip!!!
