resignation letter wording

help! im taking the plunge and resigining ive been very unhappy at my job for awhile... and its time to go now...
i work in a small company .. can someone help me with regisnation letter writing...
i know dont' name reasons u quit
but im clueless
my last day will be 2 weeks (9/21)

what i want to write will not be appropraite haha

also ill need to make a resume.. ive never had one.. and worked in a "big girl" job since 00 almost right out of high school b/c my grandma was the vp.... so im cluless as to how to write one.. ugggg
life changing things stink!!

and ps im sooo gonna cry when i hand it to him how do i stop that? im such a crier
