Semen Analysis Covered by Insurance?
Does anyone know the answer to this? DH and I are covered by Empire NYSHIP (I'm a teacher). He has an appointment with Dr. Sarah Girardi in a few weeks and the office sent him to Dr. Avner Hershlag's office from North Shore LIJ for the semen analysis. We just got the bill yesterday for the full amount and it said that insurance covered $0. Any thoughts?

I have Empire too and I'm not sure exactly...did you call them prior to the procedure? You can register on myuhc.com and see your benefits/coverages. I just tried to look for you and under Services for Treatments for Infertility it says:

Failure to notify us will result in no benefits.
Performed at a Center of Excellence with notification:
100% of eligible expenses and a $50,000 lifetime hospital / medical / surgical maximum per covered person
Travel allowance is available when traveling more than 100 miles subject to lifetime maximum
Performed by a network provider with notification:
100% of eligible expenses subject to $20 copayment and a $50,000 lifetime hospital / medical / surgical maximum per covered person
Qualified procedures:
· In-Vitro Fertilization
Gamete (GIFT)
· Zygote intrafallopian transfer procedures (ZIFT)
· Intracytoplasmic sperm injection (ICSI)
· Assisting hatching
· Microsurgical epididymal sperm aspiration (MESA)
· Testicular sperm extraction (TESE)
· Cryopreservation
· Storage of sperm, or embryos
OUT-OF-NETWORK

not sure if sperm analysis falls under this category? I would call NYSHIP

Ugh! Crap! I did NOT call before he went because I knew they were an in-network provider. I will call NYSHIP tomorrow. Thank you for your help!

Good luck! Like I said, not sure if the semen analysis falls under that category, no experience with it, but I hope they can work something out with you!
