So apparently the government thinks there are 2 of me, Update please proof my letter!
Last year I had a summons for jury duty and I served on a case no problem it was actually a nice vacation from work. Well today I get a summons again to serve jury duty this time it was mailed in my maiden name! I don't understand how this happened, I got the summons last year in my married name which I've been using since 2004. 
Update:
Ok this is the letter I'm going to send, what do you guys think? If this doesn't get me out of jury duty I don't know what will!

Dear Sir or Madam,
First, I legally changed my name from Doreen ___to Doreen ___ effective 8/04. Enclosed is a copy of my marriage certificate. Kindly update your records. Second, I recently served jury duty in 1/06. Enclosed is a copy of my juror’s proof of certificate. Furthermore you will find a copy of my daughter’s birth certificate enclosed. I am the sole caregiver for her Monday-Friday and have no one that is able to watch her during the day. I work part time at night as documented.

 Coincidentally my DH got his own summons recently so has my brother. They want all of us!
