The items that I thought was a waste was the Pack n Play(Ella only used it once) and the bottle sterilizer(I never used it I boiled her bottles the old fashion way).

Honestly for me it was the bedding. I don't use the bumpers or quilt because I worried about SIDS. At 14 months DD still wears a sleep sack to keep warm. She wriggles around so much a blanket would never stay on her anyway
The quilt is draped over a futon we have in her room. I'm hoping I'll get to use it when she transitions to the toddler bed.

This is a very hard question to answer because you'll find some people never used or hated some things, while others loved it. It's all gonna depend on you and what your baby likes. For me, I used everything and can't imagine living without it all, including the PNP, bouncy chair, boppy, swing and floor gym. The only thing I think I could have done without was the bottle sterilizer. I used it for awhile, but it was a pain and just started putting everything in the dishwasher.

diaper genie.
We used it for the first couple weeks, but when you have to empty it, the smell of poop from days ago is so rancid, I would gag. We stopped using it

drying rack for bottles, dishwasher rack for the bottles, wipes warmer, ear thermometer (the rectal is so much more accurate and u get over the fear of using it really fast...lol), newborn clothes (but i had a big baby 9lbs 2oz)

I agree with krash that depending on the baby and the set up of the house, you'll get different answers.
Some babies hate the swing, some live in it (don't do that, bad habit  ). Some families have 2 floors or are at the grandparents so much that the PNP comes in handy.
Things I found to be an incredible waste were sterilizers, wipes warmer and bottle warmers. Also while I used almost everything I registered for, I wish I was not so wrapped up in new things for the baby and let my sister give me her stuff. They use these things for such a short period of time. To have my friends spend hundreds of dollars was so wasteful.

OMG! Congrats Nic! I have no idea what rock I've been living under!  
I found the wipes warmer to be a waste. I also registered for so many different kinds of blankets and have yet to use all of the receiving blankets. So my suggestion is to not register for 50.
Other than that, I've pretty much used everything I registered for.

Everyone is different so you'll get varying responses!
I got a ton of hand me down stuff so I didn't mind that DS didn't use some things. He hated the PNP so we only used that a few times. I found a pillow was better for feeding than the boppy. I liked the NB clothes even though he was only in them for about 5 weeks; the 0-3 were just so huge on him in the beggining that they would bunch up around his chin. I think it was more annoying for me, though.
I love the drying rack for bottles (best to go to Walmart/K-Mart/Target-only $8) and the bottle warmer. He usually gets room temp bottles but if his bottle has been in the fridge I use it. It was a gift and I registered for an inexpensive one ($20 at BBB).
