Started Re-Fi and in my Good Faith Estamate includes PMI.
I applied for my Re-fi and wa told what my estimated payment would be - no mention of PMI at all. So I pay all the fees and then get my disclosures online. Honestly if PMI was mentioned initially , I would have NEVER proceeded and paid the "non-refundable" costs.
Just an FYI, at these times ask if there might be PMI in addition to your loan before you give over your credit card number.

I would totally fight that if it was not disclosed to you. Did you switch banks? Did you use a broker? Also, maybe it was already estimated in your revised figures and the GFE is now breaking it out for you?

The PMI adds $310 a month and I would only save $16 a month....
It's a new bank and definately wasn't disclosed. I'm really bummed.
