Subchorionic hematoma, possible unformed twin and hcg levels - help!

Hi. Just wondering if anyone has experience with any of these. I have all 3. I'm 7.5 weeks and about a week and a half ago I started having brown spotting. I went to the dr for an early appt on Wednesday and we saw the baby (thank god) and a flicker of heartbeat but then two other masses next to the baby (looked like 2 more empty sacs). The doctor said it's hard to tell but felt it was an unformed twin and a subchorionic hematoma. He seemed really unconcerned and said the worst that will happen is that it will make me upset because of the bleeding. He didn't prescribe rest or pelvic rest as I've now read are common.

He did my levels on Wed and again on Friday. On Wed (I was 6.5 weeks), my hcg was over 56K which apparently is high for that time, and my progesterone was 18. On Friday my hcg didn't double. It was only 71K, but my progesterone was i think 24. They said the progesterone is great and told me not to worry about the hcg because it was high and when it's high, doubling takes longer. I asked if they wanted me to come in for more blood and they said no, they'll see me this friday for an ultrasound (my original dating ultrasound appt).

I don't know if all this is really nothing to worry about or if my doctor is trying to keep me calm since there is nothing i can do about it. I didn't have any hormones tested the first time around so i really don't know.

I go from being ok to totally freaking out.

Thanks

Re: Subchorionic hematoma, possible unformed twin and hcg levels - help!

I had a SCH with this pregnancy. At 7 weeks, I stood up a felt a gush and just knew it was blood. I continued to bleed for the next 6 hours and then it turned to brown spotting. I was supposed to go for my first prenatal appointment the next day anyway and due to the bleeding and a previous ectopic, I was squeezed in for a sono. It showed a moderate sized SCH. I was put on pelvic rest, told that it could go either way and scheduled for another appointment a week later. The brown spotting stopped after another day or so. At my next appointment, the sono showed either the same or slightly smaller SCH and it was right over my cervix. The doctor did an internal exam too and said I didn't need to be on pelvic rest any more. Of course between the sono and exam I began brown spotting again that night and it lasted for a few days, but it was only when I went to the bathroom. I kept myself on pelvic rest and exercise restrictions until there was no spotting for a week. I think I had very light brown spotting once after that, but nothing else. At my 12 week sono, the SCH was completely gone.

I was told to expect bleeding especially because of the location of the SCH, but that I didn't need to call unless it was filling up a pad within an hour and/or accompanied by severe cramps. I would want the SCH measured at your next appointment and to know where it is in location to the baby. I'd take it easy and drink plenty of water.

I don't remember exactly when, but the hcg stops doubling every 48-72 hours. Yours is definitely past the point of not doubling as quickly as it does early on. I wouldn't worry about that. I hope everything goes well at your next appointment. Good luck!

I had a SCH in the beginning also. I started bleeding and they told me to go to ER bc the sono tech wasn't in at that point. I was about 8 weeks. they ended up finding SCH and then i went to the dr the next day etc. She put me on pelvic rest until about 12-13 weeks. she was close to putting my on bed rest towards the end. i was nervous about it but she said SCH is pretty common, which is good to know.

just stay hydrated and keep your feet up and try not to overdo it. they normally resolve themselves in a few weeks (either by coming out or getting reabsorbed)

i think hcg stops doubling at a certain point to be honest. so i am sure if your doctor is saying everything is fine with that, it is. :)

after 1200 you wont see the normal doubling pattern.

As far as the twin & SCH, both are pretty common. If it was a twin, it will just absorb back into your body... thats what happened with me.

Once you see a heartbeat, they rarely do beta levels. And by 6.5 weeks, doubling time would no longer be 2 days. So the increase is all they were looking for.

I had bleeding during both of my pregnancies and there was never a cause found. But now I have 2 healthy children. Hang in there! Once there's a heartbeat, it's a good sign!

I had a massive SCH with my first. I was told that anytime there was active bleeding, I was to immediately lie down. I bled off and on for weeks (4-6), but never as bad as the original bleeding (it seriously looked like a massacre had taken place in my house). It was terrifying. Thankfully, the hematoma was absorbed eventually and I had a healthy pregnancy and healthy baby.

Yes. I currently have a SCH with my triplets. It is small but I have been bleeding on and off for over a month now. Brown then back to red, then back to brown and so on. As long as the SCH is small they won't be too concerned with it and it should resolve on it's own. I'm sure if your doctor didn;t seem too concerned then it is probably a small sized SCH.

Yes, when I was pregnant with my son who is 5. Scary. I bled for several weeks. Sometimes bright red. Sometimes brown. One night, I expelled something on the toilet. Purple stringy clots. Scary but it all turned out fine. The second sac disappeared and the bleeding stopped.

Good luck!

Thank you!! Last Friday I went for my regular dating sonogram. baby measuring right on time with good heartbeat!

The first time the dr didn't appear to measure the SCH but this time he said (other dr in practice this time) it was only about 2cm. It looked a lot smaller this time in relation to the baby (not sure if just baby growth or the bleed shrinking or what). The bleeding stopped earlier this week, so I'm feeling better now.

I asked about pelvic rest and bed rest, and he said it was old school and wouldn't actually prevent a miscarriage.

I'm feeling much more positive.
