Did the DOE send you any forms or is it up to us to send in the forms. Who did you send it too? When did you submit it and how long doe sit take for them to process it?

Your school is suppose to input the hours for you. Not sure where you send it for the DOE.

i submitted mine last year. i got a packet from my payroll secretary, where i had to write in all the hours and then get the principals signature and fax it to the state. i am DOE.
