Update from my morning appointment - Now with PM update

So I spoke to the nurse this AM when I went in for blood work. I asked her why I had to decrease my dose of gonal-f and come back in again today when I was there yesterday for blood and sono. She said my estrogen went from 76 to 153 from Saturday to Monday. I have lots of small follies but nothing significant yet. She said they decreased the dose because the doctor doesn't want my estrogen going up too quickly because I might wind up with too many mature follies.

Can someone explain this again to me in simple terms? I sort of get it, but she confused me a bit...

I believe when your estragen jumps up a lot in a short period they become concerns with OHSS. They probably lowered the doese so as not to risk it.

Right. It's very common w/ someone w/ PCOS to have a lot of 'mid-range' follicles. They stay mid-range (like b/t 8-12 or so) and never get bigger- while some of the other ones do grow to mature follicles.
I had 45 total follicles on my right ovary, and over 30 on my left last cycle (the one they cancelled)- but of those 75+, he felt that only 20 or so were growing enough to be mature (or would've been had he actually triggered me).

Should I be concerned about OHSS at this point? Or do you think it's just a precaution?

No- it's just a precaution.
My 1st cycle my estrodial was over 1400 and I got OHSS but it literally doubled from 700 to over 1400 overnight if I recall correctly. They can kind of track where you're heading which is why they lessened the dose. Chat Icon Chat Icon Chat Icon
Just make sure you're drinking at least lots of water- or a 50/50 mix of gatorade all along just to be safe. It'll definitely help if you end up w/ OHSS later on in your cycle. It sounds like your dr is taking caution now so hopefully you won't. Chat Icon Chat Icon Chat Icon Chat Icon Chat Icon

I am so paranoid - Today I have a cramp on the left side of my stomach - almost like a knife pain. Now I am analyzing everything Chat Icon

It's totally normal to be paranoid - I was too. Just be cautious and call your dr if you're worried about pains that may develop. That's what you pay him for- so utilize that phone!!! Chat Icon Chat Icon Chat Icon

I felt pains also- it's taking your body which usually only produces 1 (sometimes 2) follies a month, and making your ovaries work harder to produce more- so you can expect to feel twinges, pain, stuff... HTH! Chat Icon Chat Icon Chat Icon

I just got a message that I am to go back up to 75iu of gonal- f tonight and tomorrow and return to the doctor on Thursday AM for blood and sono. I wish I knew exactly why he had me come back down and then go back up with the dosage. I HATE that it's a voicemail system and can't talk to someone directly to ask exactly what's going on Chat Icon

Your estrodial number is directly related to your follicles.
It means that the follicles are either multiplying in quantity or there is one that is growing more rapidly. In your case, they are starting to multiply.
Because you are at risk for hyperstimming they will monitor you closely and reduce the dosage periodically to slow down stimulation. They want the follicles to grow together, but they don't want too many.

If your estrodial number is in the 3000 range, then you have to worry about OHSS...at 200/300 you have no fears.

When I ended up in the hospital my last estrodial was 4600...my last stim cycle it was around 2500 and I didn't have OHSS that time.
