I did some research over the past couple of days and I wanted to share what I found out so that others could use it and save themselves some aggravation:

If you have private insurance (meaning your employer is not self funded) there is a NYS Assembly Bill #1818A that states that your ins carrier must pay for your formula. You need to purchase it yourself, but you can send in the receipts with the prescription from the Dr, a letter of medical necessity from your Dr and a claim form, and you will be reimbursed.

Neocate has a form letter you can access on their website where you just fill in your info and have your Dr sign it. Here is a link to taht website:

The letter includes and NDC (National Drug Code) number which helps the insurance identify Neocate.

Also, I found out that Wal Mart pharmacy carries Neocate and it is $31 can as opposed to $48 a can at other pharmacies, so if you do have to pay for it yourself, or even a portion of it, at least it will be a little cheaper.
