What paperwork do you have to bring to the hospital with you?
I want to make sure I have everything together in my bag so I don't forget anything...

Insurance ID card Driver's License Name, telephone number and address of DCs pediatrician Telephone number for insurance to add DC to insurance

You might want to check with your hospital.
I didn't have to bring anything other than my pediatricians name and number (since they were not affiliated with the hospital I delivered at).
I had pre-registered with the hosptial when I was 28 weeks, so they had all my information already.

I have my pediatrician sheet filled out I mailed in the paperwork already that we received from the hospital Insurance card and license I will have on hand, but DH has his as well

In my registration packet there was a pink card that I had to write my address on saying that they received it so they could mail it back to me.
As for a ped sheet not sure what that is, but it might be for a particular hospital.

I brought a copy of my insurance card, some written instructions for DH, the ped's name and number, and a copy of my pre-registration packet. I ended up not needing anything b/c they had everything on file b/c I pre-registered. My doctor's office gave me a pakcet that I had to mail to the hospital I think by the time I was 30 weeks.

Pre-registration meant nothing when I had Abby. I still had to bring copies of everything and fill out paper work while I was in full blow labor. Joy. I was lucky I could hold a freakin pen.

I made copies of all the preregistration stuff to bring with me...a coworker gave me that tip. I will also bring the ped's #, which I was also told either me or dh should call the ped anyway b/c sometimes it takes a while for the hospital to call them and they want to see the baby at least 2 times in the hospital...if i am there for only 48 hours that doesn't leave a lot of time.
