8 months when I stopped BF/pumping and ran out of frozen milk.
He was mostly BF until then.
We never heated formula - just room temperature nursery water with formula powder.

I never gave her warm formula or milk...I never wanted to start a bad habit of her only wanting warm formula. And a few weeks ago I forgot water for her powder formula and I was able to buy water out of a vending machine, it was ice cold and she didn't mind one bit!

Both of my DC were breastfed for a little but when I stated formula I heated up for my DS a couple of months, and I never heated it for my DD.

We take the chill out, which I would consider heating but it's not hot or really even warm - just not directly out of the fridge. I did it with Colin until he switched to milk at 10.5 months (and then the thought of warm milk grossed me out) and I'll probably do the same for Reid. I've tried to stick it in his mouth cold and he would have none of it.

We still do at 2 years old. DD will never drink her milk unless it is warm and we prefer the warm milk for her.
