I get mine from http://www.boomerangtags.com/
They are deeply engraved, and are guaranteed to never wear out. free shipping and you get it in just a few days. I like doing things online when I think about it, otherwise I never get around to it..

Petco and Petsmart both have machines. You pay at register they give you a token and you type the info as you want it in the machine and it engraves it and it comes out. Quick and cheap.

We got our first one on a store on eBay. A year later, we wanted something different so we got new collars and tags at 3 Dirty Dawgz

I just got a new one at Petsmart today (Huntington)
It is the coolest tag ever! It comes with a protective film to put on it so the engraving doesn't wear away
