NYCDOE- chngng ur name

im pretty sure this was already posted, but i cant find the thread.

i printed out the OP-85. is there anything else i need to do? is this going to mess up my direct deposit?

also, did youchange your name on your licenses? i have one permanent and one provisionnal. i am starting a new degree this fall.

anything else i should know/do?

Ok, so now you need to get the change of address form from the UFT and you alos need to change your address with the TRS/TDA. You can actually do this on their website, I did. Additionally, as far as your license goes...I NEVER changed my last name. Also with direct deposit..a name change has nothing to do with it. As long as you aren't changing accounts!

i don't know about the DOE name change process, but you have to change it with TRS...

I changed them on my certifications too... you do this with NYSED. go to nysed.gov and go to teachers and certification. create an online account through TEACH and it can be done there.

It is $25.00 per certificate for a name change. for your provisional, i would wait until you have permanent cert. that way you don't have to do it twice. just apply for your perm. cert. with your new name and only pay for that certificate rather than name change on provisional and then having to apply for the permanent one anyway. for your permanent cert. just do the name change.
