When did you send your paperwork into the hosptial?
Last time my doctor gave it to me about 5 months. this time they gave it to me at 10 weeks! I figured that's too early, I've been sitting on it for a few weeks and was thinking maybe I should start working on it. 5 months seems like enough time for the hosptial, right?

We actually just filled it out and plan to send it out this week, I am 30 weeks.

I have no idea! I'm almost 23 weeks, I'll have to ask them on my next appt, which will be at 25 weeks.

I never got to give mine it. They gave it to me at my first appointment and I lost the papers, so I asked for them at my 34 week appointment and I ended up having the baby at 35w1d at the hospital where I was not supposed to deliver (I was supposed to go to St Charles but ended up at Stony Brook)

Is that the maternity/labor pre cerfication paperwork for delivery?
If it is I guess I am lucky up here in CT, my dr does that for me with the insurance company who ok's the hospital.
Does the hospital have seperate paperwork?

I was never given any paperwork to take home, I just filled it out at the doctors office at my 36 week visit. Geez, I hope it was the same thing.

It's just a few quick forms, I just wanted to make sure most people did it closer to the end of their pregnancy, not this early!

I thought her question was about the paperwork that the Dr gives to the hospital where you are delivering. (basic info, its like pre-registering at the hospital)

Yup, that's it, but my doctor doesn't fill it out for me, he gave it to me. Just wondering how much lead time the hosptial needs.

I actually sent mine back as soon as I received it which was about 6 months in..
