HR people - a question

Do you have a standard letter that you give to employees when they are terminated re: continuation of health insurance? We have to let someone go and I have to give them information about state continuation (equivalent of COBRA for companies with less than 20 employees).

Our termination letter has this line in it:

You can extend this health insurance as is provided under Federal law by purchasing coverage yourself. A notice of this right is enclosed which includes an application to extend the coverage.

Then a copy of the COBRA application with the information is enclosed with it.
