Here are the requirements of the position.

I know it involves fieldwork and attending client team meetings as well as working closely with case management/social work staff.

Requirements•Experience in Home Health Care as well as Supervision/Management.
•RN with at least two years of home care or community based care experience and at least one year of supervisory or management experience.
•BSN Preferred but not required.
•Familiarity with the NYS TBI/NHTD waiver program is a plus!
•Must have car and valid driver’s license
•Proficiency in written and oral communications
•Extensive travel throughout Queens and Nassau Counties
