I ususally start with the job title I am applying for and how I found it. I then introduce myself, describing my background and education in 2-3 sentences. Then I go into further detail of why I would be a good fit for the position, specifically addressing experience and desired skills listed in the job description (usually a paragraph or 2).
Also include previous/current or desired salary range if they ask for it.

a cover letter should tell the employer something your resume doesn't. use this as your opportunity to 'wow' them....a good cover letter gets your resume opened, a good resume gets a phone call... you know the drill. i like to include words in the job description within the cover letter.
