Go through the school district.
But be forewarned they are only going to "diagnose" for behavior. He may qualify for services such as SEIT and OT.
No one will give a ADHD diagnosis that young from what I have researched/been told.
I even spoke to someone who works in the ADHD program through the hospital system I work at, and she told me she would take down my contact info and check in with me again at the end of the school year.
