3 day transfer vs 5 day,,??
Was wondering if I could get a little feed back. I just went back to my RE today to discuss our 2nd round of IVF in jan. my first IVF was in April I have PCOS, Low AMH levels .60 & a rare autoimmune disease that is currently in remission. They went pretty agressive with my first stems and I ended up getting 16 eggs and with icsi we got 7 embryos. We transferred 2 good quality embies in at day 3 transfer and got a negative so we saved up the money for a FET we put 3 in and same negative. I have 2 left but the quality on one of them isn't the best. We have decided to take more loans to pay for another full IVF because it just doesn't make sense to us to transfer the last 2 due to the high cost.
Today when we discussed my upcoming protocol my RE recommended doing a day 5 transfer this time. I'm nervous because I have read that day 5's don't usually freeze well and what if none of them make it to that point.
So my question is how do you ladies feel about day 3 vs day transfers,,,,??????

This is my 1st IVF cycle so I don't have much info about the results since I am still doing the birth control part now. Just wanted to add that my RE recommended doing the 5 day transfer saying that if there is no exact reason to do the 3 day, he would rather do 5 days since the embryos seem to be more stable at day 5 for implantation.

You can freeze on day 5 just fine. It's day-3s you can't freeze, so you don't have to worry there. But you will want to make sure your clinic does vitrification.
Growing out to day 5 is a risk because you might not have embryos make it. As a DOR girl myself, I have spent a lot of time consulting and researching this question. And this is what I've found:
* Some doctors think the uterine environment is better for embryos, so they like to get them back in faster because they think it gives them a better chance. They believe the endometrium releases hormones that help embryo growth, toxins in the air, poking and prodding, etc. Cornell thinks this way.
* Other doctors think if you have a really great lab then embryos are just as happy outside as inside, and growing to day 5 takes the mystery out of why you're not getting pregnant (i.e. if they arrest in the lab they would have arrested inside, but you'd never know that doing a day-3 tx). Truth is that a really important process happens between days 3 and 5 that a lot of decent looking day-3 embryos won't be able to pull off, so day-3 transfers can be deceiving. These doctors also have research that shows the uterus is hostile on day 3 since the embryo should technically still be in the tube at that point. CCRM thinks this way.

We have done 2 5dt and 1 3dt. From all the reading I have done I think 5dt is the best because they can better tell the quality. We have had a lot of embryos look good on day 3 and then arrest before day 5. If you have a good lab, I think day 5 is best.

This is my thought too. I've had too many miscarriages at this point, and I feel like waiting out the other 2 days may "weed out" the ones that I may have miscarried.

We just did a 5dt. I have DOR and was petrified of doing this but the dr explained the differences between 3dt and 5dt and we decided 5 dr is the best option. Like the pp said, if they all arrested by day 3, we would know more about my eggs.
With us, the 5 eggs that fertilized, none arrested but oy 2 grew to where they should be on day 5. Well, really 1. (Had early blast & morula. Morula being more of a day 4 embryo) so for me, it seems a day 5 was a better choice. Just to know where my embryos stand. On day 3, they would have all looked great (they didn't look at them. They believe in not disturbing them at all until the day of transfer-which I think is why the bottom 3 didn't arrest) but at day 5, we saw the true story.
It's a big decision and there will always be conflicting stories. But you have to do what's right for you. And if you trust your RE, do what they recommend.

Our diagnosis is unexplained. I did 2 5DT. With the first IVF I had none to freeze bc on day 5 and 6 they were still early blasts. I transferred 2 early blasts on day 5. BFN.
With IVF #2 I transferred 2 BBs on day 5. We froze one BB on day 5 and one AC on day 6. The other ones were too slow on day 6 and didn't have good potential. Chemical- low beta of 5.
Personally if you have many to choose from I would do a 5dt. If I only had 3 embryos at day 3 I would probably do a 3 day transfer.
What are the grades of the 2 you have left?

I think the number of embryos factors in, at least for me. I produce so few eggs that attempting a 5 dt has never been a consideration. I'm AMA/DOR with VERY low AMH.
Of the 2 cycles where I made it to transfer (I had another cycle that was cancelled), we only wound up with 2 embryos the first time and 1 the second time. IVF #2 : transferred 2 embryos and we have our DD from that cycle. IVF#3 : transferred the 1 and had a chemical as a result.
I subscribe to Cornell's belief that they fare better in the body than the lab. I know this is a debatable thought, but my DD is here as a result of a 3dt.
ETA: with the successful cycle, my embryos were not graded well at all, but the love of my life is next to me right now as a result.

Yes, you can freeze day 3s. I got my bfp from a day 3 transfer and I'm expecting twins.... I have 6 frozen on day 3.
I think there are benefits to both 3 and 5 day transfers. If your RE thinks day 5 would help, and you trust him, I'd go for it.
I could be wrong, but if the embryos don't look good on day 3, they would probably push up the transfer. If this isn't their standard, I'm sure it is something you can address and see what they say.

I did two IVF on day 3 and both bfn. My new dr practice only does day 5. I feel like they have a better chance in a good lab on day 5. My old practice says that are moving towards day 5. They did day 3 bc I had few embryos and maybe they would do better in natural environment. They didn't. Everyone is different but I will not do another day 3 after 2 failed IVF.
