what do you think?
Anyone with experience with 4g and mthfr stuff? If so, let me know your thoughts.
Had thrombophilia testing last year. Results:
homozygous 4G variant in the PAI-1 gene.
Heterozygous MTHFR C677 T allele
My OBGYN in partnership with a high risk perinatal specialist ran the tests and the doc put me on:
Metformin 1mg folic acid 1 baby aspirin a day
I go to RS of NY. My understanding of these issues is that it can cause implantation difficulties. When I speak with RS of NY docs, they think this is all crap and talk about studies that don't show whether metformin works.
I had a BFN Fresh IVF with two AA's transferred.
I just had FET with two AA and AB's transferred. I got pregnant and within a week, it began to fail.
I called my OBGYN and he asked if I was on lovonox from RS of NY and indicated that some docs might put me on it. RS of NY actually told me they would take me off of metformin at 6 weeks pregnant, which my OBGYN strongly opposed.
When I raised the lovonox to RS of NY, the doc went on about how "other" clinics are slapsticking medicines with no evidence that it changes anything.
So basically I know they are going to recommend another FET with no change in protocol.
I feel like I am living in two worlds with an OBGYN who is far more proactive about being aggressive and looking at everything. And RS of NY who just wants to do fertility protocols and wait for the lucky one. At the bargain price of all cash each time I pay.
Crossroads.......what to do. Lots to think about while I wait for this pregnancy to fail.

RSoNY is strongly against meds and immune testing. I had to leave in order to get the specialized tests that we need. Some Docs are strongly against lovenox and metformin while others are strongly pro. Have you consulted with other REs? To have such great embryos and still two failed cycles seems a bit off to me. If I were you, I would consult around a bit and get other opinions. You don't have to leave the practice. I consulted around, went back to them, then consulted again and left.

I live and work in Smithtown and don't want to travel to the city...ugh. Who can I see on the Island that would be good? Maybe Zapantis although I just read his office support is a nightmare. I know braverman is good although his transfers are in the city. What a nightmare to get a tank and drive my FE's to the city...I would be so worried they would get harmed.
Lillies: what if I met with braverman, got his opinion and if he recommended another medicine,, try to force RS of NY to prescribe it?

Braverman will def agree with the meds lol he's big on that so you know the protocol. He's not a RE but am immunologist and many REs do not take him seriously so I doubt you can force them to take on the protocol. What about Kofinas? LIIVF works with him to a level.

isn't kofinas a peri? or is an RE? I am going to meet with my obgyn on Saturday (dr. pillitterri) who will consult with my perinatologist (sp?) dr. elkady (he sent me to her) who did the testing and got these results.
Or, I could relax and take RS of NY's approach - "most times its the embryos fault and you have to keep trying".......ARGH!

Kofinas is a perinatologist. He tests and treats immune and thrombo issues and he's amazing. He works very closely with pillitteri actually.

Kofinas is a perinatologist. He tests and treats immune and thrombo issues and he's amazing. He works very closely with pillitteri actually.
FYI: Kofinas' brother is an RE. He subscribes to the same school of thought as his brother re: immune issues etc.

Kofinas is not in my insurance network which is why pillitterri sent me to elkady.
Where is the kofinas RE?

I have no recommendations for REs that specialize in these things, but I firmly believe you gotta do what you gotta to do. I live in upstate NY and after 2 failed IVF cycles there, we consulted with RMA in NYC. Was it a HUGE PITA to arrange things so we could do our IVF cycle in NYC? YES, but in the end, it was totally worth it.
Also, we have had to transfer DH's frozen sperm hundreds of miles! Purchase-->Albany-->NYC-->Purchase with no problems.

I would definitely see Kofinas.

I went to LI Ivf also. They put me on lovenox for borderline test results - they are thorough and believe in immune and clotting etc.

If I were you I'd start all over new doctors all around. I know it's a pain in the butt, but sometimes it's good to start fresh. A new doctor with at the records from both doctors and may see something they missed. It never hurts to get a new opinion. Good Luck!

RS of NY will not prescribe it. I left them when numerous failed attempts and I knew something more needed to be done. I suggest Braverman and Kofinas!

I already switched once from LI IVF after they screwed up too many things. What a pain in the ass! I totally hear you on the recommendation however......
I really respect my obgyn who I know texts with Kofinas and braverman all day long on his clients. Will chat with him on saturday and go from there.
