She hasn't looked well or healthy for many years. I think the last time she looked healthy was when she was pregnant with her twins. It is a shame because she is a very beautiful women and this dramatically diminishes her looks. I wonder if she cannot gain weight or has a health condition that keeps her so thin.

That is why I am thinking it's some kind of medical issue preventing her from putting on weight.
Because I can't imagine WANTING to look like this. And not only that, what level of starvation it would take to look like this on purpose. Like how many calories could you be able to eat? That has to be super super hard to maintain.
Unless she has an eating disorder, which is another possibility.
