Aggressive IUI???

Hi ladies. I've had four failed IUIs.
Try 1- clomid 50
Try 2-clomid 100
Try 3- follistim 150 and trigger shot
Try 4- follistim 300 and trigger shot
Just had an unsuccessful IVF. Three embryos put back, none took and none left to freeze. Thought that was going to be it for me because I was going to be out of sperm and money, but they did ICSI with my four eggs and then refroze the rest of my vial of sperm so I could use it again. I'm at LI IVF and they make you pay upfront for frozen embryo storage so I have a credit of $1,500. So I only have to come up with $1,500 more for one more injectible cycle to use the last of my sperm and then be done and no second baby. I have an appointment with my dr on March 15 to discuss with him but I think LI IVF tends to be conservative and not think out of the box. I'm wondering if anyone can share an aggressive IUI cycle they did that's different from what I've already done. Thanks so much.

How many follies did you have for each of your IUIs?

I'm assuming (hoping) that when you say Follistim 150 or 300 they didn't keep you on a steady dose the entire time? Doses needs to be monitored in case you need more or less throughout your cycle.

Are you triggering? If so, how many hours after trigger is the IUI?

None of my doses were changed during my cycles. My clomid cycles I got two follicles. My third IUI, 150 Follistim I think it was 3 or 4 and then on 300 I had a ton like 12 or 13 but the problem with both Follistim cycles was that I started surging on my own leaving btwn 2-4 follicles (4 is reaching) that were even big enough to be viable. I did trigger on the Follistim cycles but it was pretty mute. I got the phone calls for both to hurry up and take the ovidrel right now because you're surging and your IUI is tomorrow morning. So it was never 36 hours later it was about 20 hours later. On the Follistim my problem wasn't making follicles, it was prematurely ovulating before most follices were big enough. I would have one or two big ones at 18ish and then the rest at 13-15.

You only want 1 or 2 mature follies for an IUI. Possibly 3, but not more than that. I don't know any RE who would do an IUI with something like 10 mature follies...or you'd be like Kate Plus 8!

Well because of my age I do want more than two mature follicles. I'm sure they would have canceled the cycle if most of those were viable options but they weren't big enough to be.

I would definitely get second and third opinions. I would just hold on and not jump into it just in case another RE brings up something new

Whats your diagnosis? I agree on getting another opinion. I switched REs and it was the best decision I made.

Being single lol. I'm a single mom using donor sperm. And old age lol. I'm 39 now (38 when I started trying). My son is 4, he was easy. 1 st IUI bfn, second BFP but miscarried, 3 IUI was my son. This time around no luck. No issues that have been found.

I'm not sure of many REs who will do IUIs with more than 2 follies. Maybe 3 tops. Any more gets dangerous, regardless of your age. Remember that carrying a multiples pregnancy would also be more dangerous, especially at an older age.

Have you been tested? Is there any reason to believe why you're having a harder time getting pregnant? Are your tubes open and all hormone levels ok?

I'd give it a few more tries, probably with another RE who will more closely monitor you. My guess is your IUIs have not been timed well if you say you've been surging on your own. The goal is to have you ovulate via trigger, so the IUI can be timed to 36 hours post trigger.

You can ask about using Ganirelix. It's typically used during IVF cycles to prevent ovulating on your own and give your follies a bit more time to grow. When I got pg with DS, I used Ganirelix for 1 night to give a slower follie time to mature. I had 3 mature follies and got pg with DS. My second pregnancy, I had 2 mature follies and got pregnant with DD.

I've been tested, tubes are clear and all the bloodwork was done. And I'm monitored very closely. I'm there for daily bloodwork and sonos from day 9 or 10 onwards until the IUI. So frustrating that it's not working being so closely monitored and no issues that are keeping me from getting pregnant.

If this truly is your last cycle, I would 100% get another opinion from a different office. After 4 IUIs and an IVF all which have failed, I definitely wouldn't be putting my fate in the hands of that office. Infertility isn't black or white- a different set of eyes could be exactly what you need here.

LOL great diagnosis. I would definitely see another RE especially if this is your last shot. I would see what other testing can be done.
