Passport Questions

My passport expired this past September and I'm going to Europe for 2 weeks this summer.

What do I need to do to renew my passport with a name change?

I'm pretty sure there's a new form you have to fill out for both renewing and a name change. You can get it either on-line or at the post office (I think the Patchogue one does this) You will need to send in your original marriage certificate (don't worry, you'll get it back )

I'm sorry this is not more specific - it's no hassle, though. Make sure you send it in with MORE than enough time to get it back. (Or you can pay the extra $$$ to have it expedited)
