I use Dr. Merlis on Grand Ave. in Baldwin. He takes the Welfare Fund insurance.

Do you have an insurance card for your dental? I am so overwhelmed with all the new hire paperwork on top of lesson plans, I am not sure if I had to fill out something seperate for UFT dental.

I go to the most amazing Dentist in Smithtown. Dr. Harry Boutis, on Middle Country Road. He is amazing, and the staff is too. They take UFT, I'm a city teacher.

There is no seperate card for dental. Usually your dentist will have a claim form...or atleast mine does.
