I was told grading has to do with fragmentation and cell size and has nothing to do with egg quality. If I am not mistaken you can only tell egg quailty by PGD. IMO I wouldn't get too hung up on grading. I transfered two grade AA embies and that IVF cycle didn't work. Next cycle with lower grade (still good, AB) embies ended in DS. GOOD LUCK!!

Re: Can someone explain to me embryo grading?

Trish at RSOFNY was our embryologist and she was crazy nice. RSOFNY does the double letter grading (AA, AB, etc).

The first letter is the "grade that makes the baby". the other letter has to do with how many cells and what fragmented.

We transferred an AA an AC and a BC back, my theory (although I have no idea) is that the BC did not make it.

We did ICSI and we'll be doing the ultrascreen.. and there are any soft markers on the ultrascreen we will then think about amnio/cvs/Materniti21 testing.

Re: Can someone explain to me embryo grading?

Day 3 embryos and Day 5 embryos are graded differently. Day 5 embryos are typically graded using a number and then two letters (ex. 4 AB). The number represents the expansion grade. 4 is an expanded blastocyst. The first letter refers to what would become the baby. This is typically done on a A-D scale. A being the best - least fragmentation, equal cell size, etc. The second letter represents the grade for the placenta and follows the same rules as the first letter.

Here is a good link that explain day 5 grading. http://www.advancedfertility.com/blastocystimages.htm

A standard for day 3 grading is more difficult to find.

As a pp said, day 5 is different. You'll be dealing with day 3 grading and all clinics are a little different.

At LIIVF, they use a system
Of 1-5 to grade embryos, with a 1 being a perfect embryo with no fragmentation. 1 is perfect, 2 is very good and I think below that got into fair quality.

Ideally, by day 3, they should be 8 cells. Anywhere between 7&9 is "good".

So they told me they had 1-8 cell grade 1 and 2-8 cell grade 2s on transfer day... Does that help?

Each office has there own system. I would really ask your Dr.
I always transfered perfect embryos & took me 5 FRESH IVF' cycles. I now have twins. It's very hard to perdict.
This means having a perfect embryo, doesn't mean they will stick. Sometimes an ok looking embryo will turn into the perfect baby. Wishing you lots of luck!!

Totally agree with this poster. My AA embryos resulted in a BFN. My AB gave me DD.
