Address for DeCAP claims
Can someone give me the address to send in the DeCAP Claim forms? I remember last year the address on the forms was incorrect but I don't remember where I put the correct address. I tried calling 3xs yesterday and could not get through to a live person. Left a message and haven't heard back. Thank you in advance!
