Not that I'll ever eat there again but, Macadamia Encrusted Mahi Mahi at Mama's Fish House in Maui.
In NYC, the hangar steak at Craft is delish.

I went to a Mexican restaurant in San Francisco..real mexican, not americanized or tex mex. they had butterflied filet mignon over honey smoked black beans & corn, on a flour tortilla drizzled with Mole cheese sauce. It was Heavenly.

that's one of my favorite dishes ever!!!!!
Any dish from Emeril's New Orleans is the best dish ever. 
I went there the other day for dinner with a friend and had 2 servings of mac & cheese (they are really small) and we split the cobb salad with steak

I was in Jamaica at an Italian restaurant, if you can believe that. I can't remember what it was but it was some chicken dish that was out of this world.
Also, another weird one...the best Japanese food I've ever had was at a resort in the Dominican Republic! They had about 8 restaurants at this resort with all kinds of different cuisines. They had chefs from the actual countries making the food.

The Peanut Butter Pie at Nola ( one of Emeril's restaurants in New Orleans.
I also had parmesan grits ( amazing!) with that meal & between the 2 , my boss & I didn't stop talking about that meal for YEARS!
And he was already to hate it b/c he is not a big Emeril fan.

It was a special at Cascina. It was a tilapia, but what was so good was that it was in this pureed pepper sauce..mmmm

Entree: Chateaubriand for 2 from Delmonico's Steakhouse in Las Vegas (another Emeril restaurant)
Dessert: That's a hard one, I love all desserts. I think it's a tie between White Chocolate Bread Pudding from Mim's in Roslyn and the Chocolate Stratosphere Dessert from the Top of the World in Las Vegas.

I had a delicious fruitti di mare (sp?) at Frank in NYC -- 2nd Ave and 5th or 6th St. or thereabouts.
Just about anything at Bravo Nader in Huntington is excellent!

I had a Gulf shrimp pizza appetizer there that was one of the best things I've ever had.
Chateau Briand with bleu cheese scalloped potatoes at the Four Seasons

Fried plantains, mole sauce and sour cream at Zarela's in NYC
I'm sure there ar more..but that's all I can think about right now..

If you go to Carmines in midtown or uptown Manhattan...you have to try their Chocolate Bread Pudding..it's to die for!!
