I have ghi but our prescription plan is caremark... it is through the pba. they cover 60%. I had to get a letter of medical necessity from the pediatrician, and submit it, along with a written request for coverage, to the pba.

The Empire plan covered allimentum 100%. Only needed the pediatrician to send an RX for it.

mine wasn't covered at all because it was RTF and they only covered powder!!

My son was on Nutramigen and it was covered in full, except for a $35.00 co-pay per month (where they really gave me almost 2 months worth- he was on powder).
I have GHI, but my Rx insurance is The Welfare Fun - NYC teachers.

Each plan will be different. Your employer dictates what your plan includes and excludes. The best bet is to call the ins co directly and ask. DHs (United) excluded it outright no matter what. Mine (Aetna) paid after you tried soy, goat and a bunch of other stuff. My ins would cost the same to add the girls than it would for formula.

Empire covers 100% for RTF for us. They originally sent powder but I called and requested they send RTF since I had to add cereal to thicken and it was too much powder to mix well in a bottle. They didn't have a problem changing it for me.

This for us as well. It's been a Godsend! Ey deliver automatically a months supply. If I am ready before they ship, I just call and request earlier ship date.

My son was on elecare the other prescription formula and Empire covered it. We had a prescription from the Ped and his GI doctor.
