Authorization for DC's Medical Treatment?
Anybody ever write up an authorization for a family member/babysitter watching your DC?
My DH's aunt mentioned this to us. Apparently she was watching her grandson while her dtr and son went on a trip and she had to take him to the doctor. The doctor refused to treat the baby in her care w/o a letter/form of authorization from the parents.
They live in Massachusets, so I don't know if their state requires that, but I was wondering if any of you girls have done such a thing?
My DH and I were wondering how we would go about that since our parents will definitely be watching our DD for at least a few days at a time when we go on vacations and such.

this happened to me too. fortunately i had written a note. it was when i waas on vacation and i had it notorized.

Because I am sick I talked wit my doc about this. They had a form that I had to fill out and they kept in DS file

Daycares have forms for this all of the time. If you want I can get one scanned & sent it to you.

I know I filled out something before regarding medical decisions but I can't find it.
I found this one online from a Medical Center which I think it pretty good:
