I def think it depends on the company and the culture.
biz casual can mean something completely different at a bank or Wall St firm than a "mom and pop" company.
to me it means dress pants and a nice top or blouse with nice shoes

my office is business casual. for me, it means a shirt with a collar and dress pants. and closed toe shoes.

My office is also business casual. We wear nice dress pants, or skirts for the ladies. A nice top. And we're allowed to wear open-toed shoes. During the summer it's pretty lenient too, we're allowed to wear capris and nice sandals.
Honestly, I worry about the fashion-police there more than the actual dress code!!
