That happened to me too. Mine came back unmet for mentoring. It's because the district you were mentored in never sent in the paperwork. I had to go back to the district I was mentored in and get a letter saying I was mentored and mail it to albany.
I don't know about how to get the extension, sorry.

UGHHH look at this I found on their site:
Currently, we are not accepting applications for re-issuance of an expiring Initial certificate. Please check this web page regularly to learn more about when you may apply for the re-issuance of the Initial certificate.
So now what!

you can apply for the certificates and extensions online. the extension costs $50 i believe. good luck!

This is what I had to do as well. I am pretty sure NYC also has to have this documentation as everyone in the state needs to be mentored their first year of teaching.
If you get in the paperwork ASAP, you should be fine and not need an extension. They will just backdate your certificate if necessary.
