Changing name on passport
I know I have to send them my original marriage license and pay a fee. Where can I get the form? DH and I are going to Ireland in May and I need to do this so I dont forget!!

I went to the USPO and got the application from them, filled it out, submitted it with a new photo and voila, I got it in 6 weeks.

I am actually doing this this weekend! A bunch of post offices are having an event this Saturday where you can BRING IN your forms and do not have to mail them. You can even have pics taken there. I'm going to the Islip P.O. I feel much safer doing it this way than mailing everything.

Really? How do u know this? I live in Bay Shore so it would be easy for me to go to the Islip PO

I had to do the same thing. I downloaded a copy of the form off the internet. Here's the site: US Passport Name Change
I also got a COPY of my marriage license from town hall. It has the raised seal on it and was acceptable to submit for the passport name change. I didn not want to send my original license in case it didn't get returned to me. (It's supposed to be, but you never know.)
I think I had to pay $67 for the name change, submit my current passport, take new passport photos, and fill out the form.

This form says it is for those who are changing name within 1 year of issue date on passport. If it is longer, it is this form

My mommy told me. I'll try to find online.
I couldn't find any info, but this is their number: (631)581-7931. Call and ask about the passport event this Saturday. I also saw a sign hanging last time I went. I think it was in the paper too. The Bay Shore PO might be doing it too.

You cant send a copy. They need the real one with the stamp. I did it last time and got it back no problem

You cant send a copy. They need the real one with the stamp. I did it last time and got it back no problem a copy with a raised seal is legit. I checked into it beforehand and it worked. I had no problems whatsoever and received my new passport with the name change in about 2 1/2 - 3 weeks
you have to get a copy from the town hall, you can’t photocopy the original.

Ah ok I sent my last time and it was fine...so it doesnt bother me
I put it in a big envelop enad write do not bend--and they sent it back in the same size envelope

The U.S. Dept. of State website has the forms. You can download them and send them. I would try there. When I went to the post office, the person gave me the wrong form!
There was nothing on it that distinguished exactly what it should be used for, but it got sent back to me--with no further instructions and without my ID!!! Then they lost my file for awhile because someone entered the wrong date of birth and no one could tell me what was happening. It was a PITA. But it's done, and this happens to very few people as far as I can tell.
But I wouldn't trust a postal employee to give me the correct form again.
