Chocolate/Cookie Favors-B-day
Looking for company that can personalize pieces of chocolate (mini candy bar) or even cookies for my DD birthday (her name/date).
Can anyone recommend a candy shop or bakery?

I got DS's cookies from cookiesbydesign.com (they have a website and store locations). They'll do anything you want.

My MIL...seriously. She doesn't have her own company, but she always does family showers for stuff exactly like that. She is the master at chocolate bars and personalized wrappers.
Let me know if you're interested and I'll see if she is (this isn't a business, but I tell her all the time it should be!).

You can try this place in Huntington:
Chip 'n Dipped 470-2579
I am not sure if they do the lollipops, but I thought they had it at a street fair I saw them at. I had their business card in my drawer. Let me know if it works out for you
