Here are the old & new dates:
Old Dates:
2011: 10/2-10/8, 10/23-11/5, 11/13-11/19, 11/27-12/3, 12/11-12/17
2012: 1/8-1/14, 1/22-2/4, 2/12-2/18, 3/4-3/10 and 3/25-3/31
New Dates:
2011 - 10/1-10/6, 10/22-11/3, 11/12-11/17, 11/26-12/1, 12/10-12/15
2012 - 1/8-1/12, 1/21-2/2, 2/11-2/16, 3/3-3/8, 3/24-3/29

All the Disney resorts based on availability. Sometimes the free dining promotion sells out for a particular resort or room category. You pay rack rate for the rooms but dining is included. The savings vary based on the resort and number of people in your party.
I believe the value resorts get the free quick service plan and the moderates & deluxes get the regular dining plan. You can pay the difference to upgrade your plan if you want.

I don't have anything booked. I just browse around on travel forums. 
You can book through Disney's website or use a disney specialist travel agent. In the past I had a better experience using a travel agent than Disney. There's a Mouse Tales travel vendor on LIF that can help you if you decide to go the travel agent route.

They changed the dates and mine isn't included anymore. i am glad i got it adjusted before they changed it!
