Do I need to leave a letter/note in case
my mom and MIL need to take my DC to the dr/hospital? I'm leaving for South Beach tomorrow and I'm not sure what I need to do. I've never left them before and I'm really nervous.
Should I leave a note in case the kids get sick? What does it need to say?

yes I believe so. Unless a child is 17 or 18 I do not remember you need a note. Write one stating all medical needs are left up to so and so due to certain circumstances. They are techically your childs guardian at that certain point. If the hospital does something you do not approve of I believe they can get in trouble.

When I've gone away without the kids, I left my parents with an "authorization for medical treatment".
