Does your Ped charge a fee for.....
Camp/School forms to be filled out. My Ped charges $3 per form and they will not fax it out. You can either pick-up or give them a S.A.S.E.

I don't know about the fee since I haven't dropped the form off yet, but I know that they won't fax anything to the school, they said it's a HIPPA violation.

My practice will only fill out three per child, per year. Then they charge a fee. I think most practices are now doing this.

Mine didn't. I had them fill out Andrew's school forms at his well visit and I didn't get charged. Not sure if I would have otherwise.

My PEd. provides a form. IF for some reason that form is not acceptable and you need them to fill forms out it's a $10 fee.
