These are what I keep in mine at all times:
- diapers
- wipes
- poop bags to dispose poop diapers in
- change of outfit for DS
- extra onesie
- Ouch Pouch that contains:
-Neosporin
-Band-aids
-Bottle of aspirin
-antiseptic wipes
-Neosporin wound cleaner
- extra pacifier
- pen and pad of paper
- mints
- Diaper cream
- hair tie
- a toy for DS
- a change of shirt for myself. I did this in the begining when DS was born, spit does happen 
- hand sanitizer

To add to the above list:
Bottles, powdered formula and bottled water or an insulated bad with RTF formula - if formula feeding.
Cover for breastfeeding or cooler bag with bottles of pumped milk
teething ring, (or other teething remedy or toy) burp cloth
