can anyone explain to me what I need to do? Who I need to contact? Forms I need, etc.?

it all depedns on you rcompany's policy.
I just checked into mine today and I do not have to do anythign until 2 weeks before my due date or sooner if I have complications and have to go out early.
Check with your benefits person.

It depends on who your company has disability insurance through - it's different than your health insurance. My company uses MetLife and I contacted them as soon as my Dr. mentioned putting me out a little early. They sent me the form which I gave to my Drs. office to put in my file until the date that my Dr. signs me out. They will then complete everything and send it back to MetLife.

It depends on your HR's policies and programs. Is there anyone you work with who recently went on a maternity leave? I was the first one in my co-workers to have a baby under our company's new policies, so I saved everything HR and the disability people gave me - then I shared it with the women who went out after I did. Maybe someone can share this info with you if your HR team isn't forthcoming or easy to talk to?
