filing for disability
can someone explain how this works and where/when to file? I will be out of work for a month and need to file but no one seems to want to give me any answers or help at my job. thanks!

You need to file when you are unable to work anymore. I filed once my doctor told me no more work while I was pregnant. You bring your dr the form and they fill it out, then you bring it to your job and HR fills out the rest. You mail it in and they will pay you from the day that you were declared unable to work by the dr.

My HR dept sent me the papers while I was out sick. I filled them out myself and I was given one form for the dr to fill out. I returned all forms to HR and got a check not too soon after. It came very quickly.

Just google short term disability form ny and you will find it. Fill out your portion and then bring it to your dr to complete.
