Yes. You choose the agency that does the initial evaluation and that provides services. You will do this when you fill out the initial paperwork at the county. I don't have any recommendations I randomly choose from the list.

Re: going to get an eval
I chose Kidz Therapies for the initial evals. I got lots of recs for them. No complaints. DS had a SEIT from the agency who was wonderful.
When I needed to find an actual OT though that proved to be challenging because services were approved in the middle of the school year.
2 years in a row we've had someone from NY Therapy Services which I think is the biggest provider in the area, and to be honest, I haven't been impressed with either therapist.
The first was a major tool who was always canceling or changing her schedule at the last minute to suit her. I really feel she was a waste of tax payer money and DS got nothing out of the sessions. The second, I never hear from. Although DS teacher provides the positive feedback.
As someone who works in a similar field I don't think I should have to hunt a therapist down....it's their job to reach out and communicate.
I'm sure the other ladies will have some good recs for you.

going to get an eval
We use all about kids and have three therapists come to the house (2 aba and 1 speech). I love 2 of the 3 of them, and the 3rd isn't bad other than she tends to be a little late sometimes ( we have sessions at 9 am and she's not a very efficient morning person).
My son has been improving greatly over the last two months of services and I am happy overall with them.
I've also heard great things about kidz therapy but I haven't personally used them.

Re: going to get an eval
I also selected all about kids because my sister used them with my niece (speech therapy). Currently though I have been waiting to start ABA therapy for DS (therapy can't find a team leader). In the meantime he receives Speech 2x a week through them and special instruction and so far I have no complaints about the therapists.
ETA once you call EI, someone from the county will contact you for an initial meeting, then you pick the agency, then the agency will contact you to make an appt to send evaluators to your home to evaluate your LO. Good luck!
