How do you dress on the first interview with a recruiter?
Suit it up or just professional attire?

I, personally, suit up. You only get one chance to make a first impression and you want the recruiter to feel comfortable sending you out on interviews.

Definitely a suit. I was a recruiter for a short period of time and when someone came in wearing a suit, it gave me confidence they would be properly dressed for interviews with our clients. On a slow day I would have 4-6 interviews a day and this was way before the mess with the economy. You want to do everything you can to stand out as being reliable and presentable.
