I interviewed for a job in Nov (last year) and turned it down, how can I reach out to see if it's still available??

I interviewed for a job in Nov (last year) and turned it down, how can I reach out to see if it's still available??
The BIGGEST reason I turned it down was that I was living in Queens and the job was going to be in Lake Grove (I went into the interview with the impression it was going to be in Garden City).
I also turned it down b/c I interviewed for a manager position and they did a "bate and switch" on me saying it was for a sales floor position starting at $10/hr plus commission. DH said "H*ll no"-- note: I did NOT say that to the person I interviewed with.
I now am living in Riverhead and almost desperate for a job- how can I reach out to see if that job is still available? I am almost certain that I'd have to re-apply and re-interview as it's been more than 6 mos.
