I am almost positive you do. We have a wood burning and DH wanted to convert it to gas and I knew it wouldn't be an issue because it is an insert and they use the existing chimney.
You many be thinking of the electric ones. Those just get plugged in.

the answer is yes/no.
There are ventless fireplaces that use the air in the room. I wouldn't recommend those particularly.
you can also get direct vent gas fireplaces you don't need a real chimney, you do need to vent it to the outside with a pipe. if you put it on an exterior wall, you can just go through the wall there.

You would need to vent it to the outside through a vent, not a chimney.

ok this is what i was thinking of.
Does anyone know 'approx' how much this might run? Also i keep reading about fireplace units but mantles are separate? Would a fireplace store help me with all of this?

i love this fireplace with the built-ins. notice the herringbone pattern. if you do get a fireplace with no built ins, i like the look of the fireplace being bumped out from the rest of the wall (like the picture)
here's another...

Re: if u have a gas fireplace, or know anything about them...
Price will vary. Its hard to say for a number of reasons
1) cost of the actual unit. This can run from $2K on up and you can spend 6-10 if you really want. 2) cost to install the unit with venting there is how far the vent run is, running electric to it, and running gas to it. 3) finishing the thing off- repairing the wall damage from runniing the wires, and gas and building whtever for the surround, whether buying something prefab, or building something.
we did ours as part of a major remodel, so some things we didn't have a price for, it was part of a larger big.
Plumbing, electric and framing were part of other costs, so I don't knwo how much those really run.
we bought a unit for about 4K that included the unit, vent piping and "install" of the unit and venting with the plumbing and electric seperate.
the framing we bumped into the room and also hung out the back of the house. the bump in the room we covered with cement board and had a mason install ledgestone to the ceiling. Ledgestone ran about $1800
it was an expensive project, but we love the look. you can definately do something cheaper, but then again you still need plumbing run. I'd talk to either a plumber or a fireplace store

Depends on the unit, we have a 99% Fuel efficiency so we did not need a chimney or vent.

We have a gas fireplace, and it is vented to through the wall. No Chimney needed.

Re: if u have a gas fireplace, or know anything about them...
Sorry I do not know, this was in te house when I bought it. My brother put one in himself. Had existing gas line in house and ran it, then bought insert himself...I know that doesn't help. SO sorry.
I would link you'd need plumber to extend the gas line, and fireplace company to put insert in and vent. However, I am sure you could find a knowledgeable handyman that could do it all.
Good luck

Re: if u have a gas fireplace, or know anything about them...
we had a ventless gas fireplace in our last house. There was no vent or chimney to the outside - I loved it. It threw out so much heat!
Its cost ~$1200K for the fireplace box/mantel & insert. We already had gas in the house and extended the gas line ourselves saving us the plumbing cost ($700-1000?).
you can kinda see it here:
