anyone have empire ppo i cant find any information if fertility treatments is covered by my plan

you should call them and find out exactly what is covered.
All IF coverage is not equal, it's what the employer chooses to have on the plan

I have Empire , and I am covered up to $50,000 max. life. I would call and ask about the fertility benefits.

I have empire bcbs ppo. But I believe the IF benefits vary depending upon what your company chooses to include in their insurance package. With mine I have a $20,000 lifetime cap in/out of network

I have Empire bcbs ppo and my coverage is different than this. You have to call to find out exactly what is covered, as there are different plans.

Call the number on the back of your card. Ask them to outline your coverage and send you written info.
I have Empire, but it's the NYS employee plan, COMPLETELY different from Empire BC/BS.
Be sure to ask them if IUIs, diagnostic appts., meds are covered under your regular medical insurance (mine are) and exactly what your infertility coverage extends to. DEFINITELY get it in writing. It can be very confusing.
Every employer plan is different, you have to give them your specific info to get the info that is pertinent to you in particular.
