Interested in opinions...
My admin is out, possibly going out on disability. Today I had to hunt through her desk to find vacation carryovers that were never processed. I found a boatload of them that weren't done. In the stack of them was also a Bereavement Request form for my admin for the time she took off for her boyfriend (practially fiance)'s aunt's funeral. The form had the correct date she took off, but had my signature on it. Only it wasn't my signature. It was a forged signature.
There's no reason for her to forge my name. I had given her the day off. So what gives?

She should not have done that.
Oh definitely not. But why would she do it? I'm trying to figure it out. I would've signed the form.

Do you have a Stamped signature? Did she do this after you gave the Ok? I would def take this up w/her if you are uneasy w/this! Good Luck

That is SO wrong IMO.
I had something sorta the same happen at my work. Before I left on Maternity Leave I filled out the paperwork for direct deposit and yet it never went through. When I got back in December I started hounding our payroll secretary about it and she said she'd look into it. In march (still no direct deposit, even to this day) I got something back from the main offices about the direct deposit, some numbers were off and the paperwork was re-filled out and my signature was signed.
I'm thinking she probably lost the paperwork and then resubmitted it when I was asking about it, but didn't want me to know she misplaced info with my account number on it so she signed it herself.
The scary part is that she somehow didn't lose my account number, but lost the origional paper!
She actually left for medical reasons right before I got the envelope, had a triple bi-pas, so I never did get to speak with her about it.

No she tried to forge my name. I have no stamp for my signature. I never gave her the ok to sign my name because I wouldnt' think payroll would accept a form that she signed for me.
Guess I'll be checking her attendance tomorrow. Can't I just have a normal person working for me? It's like I never know what's the truth with her. I wish I could just fire her for telling lies. She doesn't really lie straight out about work - she's vague enough to be able to chalkt hings up to misunderstandings. She definitely lies straight out about her personal life.

That is totally unacceptable! Forgery is serious business. I would definitely review her attendance and if things don't jive, talk to her about it.

Whatever her reason is i would definately feel violated and distrusting of her in the future.I would also check out to see if she was pulling other stuff like this in the past. It's very unethical in my opinion.

perhaps u were not there when she needed the signature and since she had ur permission for the day she just signed ur name---did u sign it at all prior and maybe THAT copy got lost or she misplaced it so she felt dumb asking for u to sign it again... i am the assistant at my job and i have liberty to sign the name of all the pple i work for if they r not there dont jump to conclusions, if she had the day off i dont think its much to be concerned about

I was thinking this too. Maybe you were not available when she needed/wanted the form signed and since you OK'd the time maybe she thought it didn't matter.
Do you ever allow her to sign your name in other situations?

I must agree but I'd still speak to her about it anyway. Even if you had granted the day off for her, she'd never be signing your name. She's not doing forgery with intent to defraud but she's assumming your identity in a way by signing your name.

Even if you were out of the office when she needed your signature she could have at least called you and said, "I know you are not in the office right now but I know this form needs to be in by the end of today. What should I do because I realize it needs your signature on it?"
Based upon ALL of the posts and all of the stories you have told, she seems disturbed or at least a habitual liar. This one incident is bad enough but when you put them all together she is "off her rocker".
I would say you need to at lthe very least question her point blank about all of it.

Just another opinion....I think some people don't think forging someone's name to something is a big deal. I have been asked to do it in the workplace and refused. When I said I wouldn't do it, the person got all huffy and said it wasn't a big deal, to which I replied, "well, if it isn't, why don't YOU sign the form?"
I also had an ATTORNEY boss who signed my name to a legal document without my permission simply so it would have the "secretary signature" instead of his, to save face. I told him not to do it again and if I was ever asked about it, I wouldn't lie.
She may not be too together, but she might simply not see it as a big deal. I would still address it with her.

It's one thing if you had given her the permission to sign - a lot of admins sign for their bosses. But, if you never did then yes, I would be upset.
