Just a reminder for those having school forms filled out by the doctor's office

Please make sure you double check the info on them. They filled my son's out with another patient's info. There are multiple patients at the practice with the same name. I had already filled in my son's birth date on the form but they still pulled the wrong file. Not only were all the immunization dates wrong, but DS has a severe milk allergy and they filled it out no allergies/no dietary restrictions.

good thing you caught it. I remember going over DS very carefully too since I thought they gave me DD's info!
