Does Your Ped. Charge To Fill Out Forms?
Nicholas had a doctor's appointment for a med check, issues with his weight, etc. So I needed the school form filled out also, and an RX for OT. They charged $5 to fill out the form with his height (which they wrote wrong), weight, immunizations, etc. I could see if I mailed the form in and they had to maybe mail it back or do extra work or something, but this was a scheduled visit

If it's outside of an annual visit, I believe there is a charge. I've never been charged as I always ask for a copy of their latest immunizations & schedule their annuals with any forms I need.

Oh, this is what I should do since I just got a form from daycare and we have already been to the ped's office 3 times this month for both DC's. Our next appt. is in Sept. and I told them, sorry, but the forms will be a few weeks late.

Some charge and some don't charge. Ours didn't charge for DS's daycare forms.
Looking from the pediatrician's point-of-view, in August, almost every patient is coming in with paperwork and it takes them too much time to fill it all out...so they charge!

Nope. I get his school medical forms filled out and they just ask me to leave them and they mail them home to me when they're done.

Yes, they charge $10 per kid each year which covers all the forms.

I just got a recent letter from my office saying that you can pay a yearly fee of $40 for all forms you may have to get filled out. Otherwise I think they are $15 a pop or something like that. I am going on Saturday and will probably fork over the $40

I actually have no idea about this. I should ask.
I will say that my OB charged me $25 to have them fill out my disability papers last year though.

I haven't needed to use it yet but my ped has a sign that they charge $5 for each school/camp etc form that they fill out.

No. I didn't know they could even do that. Ours just filled out one today thankfully free of charge! That seems cheap to do that

My office has an annual fee which they will fill out forms, call in prescriptions (otherwise you have to pick it up at the office). I have not joined yet, but it is something like 40 a kid with a max of 100 for the family.
