Also, Doug has since had cosmetic surgery to remove the 2 big moles on the side of his face (he had bandages on a few weeks ago when they saw him).
I don't watch the show but did a google search of them.
Looks like he did more than have the moles removed. But I'm not sure what. He looks like a completely different person.

It almost looks like his chin is shorter or something now.
