Medical Clearance form Li ivf
Did anyone else need to get this form done? I start injection tomorrow and they just told me I need this form filled out before retrieval which gives me some time but I don't really have a primary dr I go to so was wondering if my OB can fill it out? I already have a call in to my ob but I have a feeling I'm going to need to go see a primary care physician which I haven't seen in 3 years so who knows if they will even see me right away. I hate last minute things!

I had to get a letter from a maternal fetal medicine practice that I had used back in 2004 before starting IVF at North Shore (which made no sense since I had a dd in 2007 and didn't see a MFM during that pregnancy) but they wouldn't start anything until I had it.
Once they ask for a medical clearance it's usually from your PCP.

I didn't go to LI IVF but we couldn't start IVF unless i had a physical in the last 12 months, with the form filled out by my PCP. I'd call and get an appt with a PCP asap. I'm sure someone can squeeze you in.

Thanks! I was about to call my primary when my OB called and said could fill it out for me because I was just there recently. I'm going tonight to get it done. I wish I had known I needed to this sooner, at the 11th hour I was scrambling to see who could get me in but thankfully it seemed to have worked out.

What's funny is, I've been back doing treatments since June. You would think I would have needed to do it then. Oh well, at least it's handled! Thanks again!

Yep, LIIVF required one due to my weight/BMI which put me in a higher risk category for any surgical procedure requiring anesthesia. I just went to my primary for a routine checkup visit and had him fill out the form -- he did an EKG, some lung capacity test, and over all bodily physical. If I remember correctly, if you require more than 1 cycle, you'll have to get this filled out each time by a doc.
